On your first visit to the Clinic, you have the opportunity to choose the physician you wish to see on a regular basis. On those rare occasions when your physician is not available when you need care, one of our other fine providers will initiate or continue care.
When you arrive at the clinic, please check in using one of the kiosk stations. Before your first appointment, you will receive a text message with a link to complete all required paperwork Please complete this before your visit to help keep your records accurate and up-to-date and to prevent any delays with your appointment.
Our staff will be pleased to discuss our fees with you at any time. We are contracted with most insurance plans and when you present your insurance card to the front desk at the time of your appointment we can file a claim on your behalf.
Out of concern for general health and cleanliness, we ask that you not smoke, vape, or bring food and/or drinks into the Clinic.

Because we schedule a limited number of appointments each week, we need to be able to depend on these appointments to successfully operate our practice. Therefore, we have established the following office policy for missed appointments in order to improve our care for you and other patients.
A 24-hour notice must be given for all cancelled/rescheduled appointments so that others may be scheduled in your place. If you are running late and we have an opening, we will do our best to see you. Arriving more than 20 minutes late for, or not showing up to, an appointment will be considered a missed appointment. The fee for a late cancellation or missed appointment is $25. The fee must be paid before you can schedule another appointment.
Payment is due at the start of each clinic visit. We accept cash, checks, and major credit cards. Copays, deductibles, and coinsurance must be collected in full at the time of service, as required. For any charges not covered by insurance, or for patients who are self-pay, we accept cash, checks, Visa, Mastercard, and Discover.
We understand that temporary financial difficulties may occur, and we encourage you to contact our billing and insurance department so we can help you manage your account.
Your medical records are always completely confidential. We will not disclose them without your permission. If a non-VWCA physician refers you to us, we normally forward a report of our findings and recommendations to that referring physician. You must sign a form releasing information to third parties (e.g., insurance companies) authorizing release of this confidential information.
